Provide Case Management and Service Coordination to the parent participants of the Family Center; assist the Director in planning and implementing the Program Services Proposal and budget.  This position interacts with staff, participants, and community representatives.

Essential Functions (specific duties include – but are not limited to):

  • Provide service coordination for all center-based participants.
  • Maintain individual family files based on Maryland Family Network (MFN) guidelines and containing all required MFN paperwork.
  • Work one-on-one with participants to complete initial paperwork, including life skills assessment tools, and assist participants with generating personalized goal plans. These plans will be updated every three months.
  • Provide crisis intervention services when needed, and links participants with the appropriate community resources to meet their needs.
  • Plan and facilitate classes and workshops that focus on health, job readiness, and general life skills.
  • Attend various meetings with community representatives such as: Carroll Community College and the Safe Kids Coalition.
  • Serve as an additional driver when assistance is needed in picking up or taking home families.
  • Oversee the Center’s Emergency Needs Closet, the Diaper Distribution program, and setting up guest speakers.

Knowledge, Skills and Abilities:

Candidates for this position must possess the following skills: ability to take a leadership role; excellent organizational skills; excellent record-keeping and file-maintenance skills; knowledge of community resources and how to access them; knowledge and experience in working with parents of young children; effective written and verbal skills; ability to work with individuals, families and groups; and general counseling skills.

Educational Requirements:

Candidates are required to have a Bachelor’s Degree in Human Services, Social Services or Human Development and a minimum of two years’ experience in direct service delivery.  Candidates are required to have a valid driver’s license and will be subject to the “Criminal Record Check” process.  Proof of current automobile insurance at hire and annually.

Supervisory Responsibilities:   None

Work Environment:  Contact with all levels of personnel within the organization, participants in the Family Center and their children.  This position also interacts with community partners and the community at-large for outreach activities.

Position Type / Expected Hours of Work:  Normal program hours, with some flexibility according to the Center’s needs; some evening hours may be required.

Travel Requirements:  Occasional travel to meetings and client transportation may be required. Staff of the Family Center attend conferences and trainings sponsored by the Maryland Family Network.  Some of these activities may require travel outside the County.
